File the Articles of Organization
Illinois creates an LLC through Form LLC-5.5, the Articles of Organization, filed with the Secretary of State for a $150 state fee.
What Form LLC-5.5 asks for
The form covers the LLC name, the address of the principal place of business, the registered agent and registered office, whether the LLC is managed by members or by managers, and the names and addresses of the initial managers or members depending on that choice.

Fee and filing route
The filing fee is $150. Illinois offers online filing, plus mail and in-person filing in Springfield or Chicago. Expedited service is available but must be requested in person rather than by mail.
Keep the filed Articles and your file number on hand. Banks, the Department of Revenue and future filings with the Secretary of State all reference it.
After approval
Adopt an operating agreement even though Illinois does not require you to file one with the state. It sets out ownership, voting and profit shares, and banks generally ask to see it when opening an account.
